Important Announcement:
My outpatient clincal practice closed April 11,
2008 (last updated 4/13/08) I'll be working on transitional issues for my patients through the end of the month. This includes taking care of dealing with the results of labs ordered at the end of my office practice, and finishing up referrals. My main office line will continue to connect to a message line, though you might find this web site to be a better resource. Argyll Medical Group is continuing to look for a physican to replace my position, and Enloe hospital has been providing assistance as well. Hopefully a new physician will be ready to start in early August of this year. I suspect that the majority of my patients will end up continuing with Argyll Medical Group and the new provider if one is found soon, but until there are specifics of who will take my place and when they will start we do need to discuss alternate sources for care. For the most up to date information on my practice and about other local doctors who may be able to accept you as a patient go to the frequently asked questions page. In general I've found my web site to have information that is more up to date than the information from the medical society. A release of information form to have your records sent to a provider outside Argyll Medical Group is available here. If you do not have the Adobe Acrobat reader to open the release of information file you may download it from the web by clicking on its Icon to the left of this column. You may pick up a release of information in person at my office until it closes, or you may send a self addressed stamped envelope to the address below and a copy will be mailed to you. Do not send record requests to my office at Argyll Medical Group as I will not be maintaining office staff there to process those requests. You may incur an additional charge for record release requests sent to Argyll Medical Group. There is no charge for records sent directly to a new provider. There is a $20 charge per individual (requests for additional family members -at the same time- processed at no additional charge). Records come on a CD rom readable with the Adobe reader. I have set up a private mail box service for business mail related to my practice. Requests for release of your records to a new treating provider should be sent to Kevin Brown M.D. 702 Mangrove Ave., #341 Chico, CA 95926 .
